The Bastrop County Cares community engagement model is structured around Collaborative Teams, Task Forces, and a Leadership Council, all dedicated to addressing the social determinants of health through a collective impact framework.

Economic Stability
What is it: Economic Stability refers to having the financial resources needed to meet basic needs, such as food, housing, and healthcare. In terms of the Social Determinants of Health, it highlights the impact of stable employment, livable wages, and financial security on overall well-being. Economic stability reduces stress, improves access to essential services, and provides a foundation for individuals and families to thrive.
Goal: Help earn steady income to meet health needs.

Education
What is it: Access to quality education—starting with early childhood programs and extending through higher education—helps individuals build essential skills, secure better employment, and improve health outcomes. Educational attainment not only empowers individuals but also strengthens communities, creating pathways to equity and brighter futures for all.
Goal: Increase educational opportunities and help children and adolescents do well in school.

Thriving Communities
What is it: Thriving communities, as a key Social Determinant of Health, foster safe, connected, and inclusive environments where people have access to resources, social support, and opportunities to grow. Strong neighborhoods promote well-being by ensuring economic stability, quality education, healthcare, and a sense of belonging for all residents.
Goal: Increase social connection to create thriving neighborhoods.
