Change Agents (CAs) are trained to help their neighbors find access to the health resources they need. When people are connected to their community and its resources, they are better able to thrive.
Community Health Workers are trained to help their neighbors find access to the health resources they need.
Build Trust
CAs play different roles in the community and use their health education training to provide information and assistance.
In-Person Help
CAs work with the community to identify ways to improve mental and physical health.
Navigate the Healthcare System
CAs help people make appointments for their needs: health clinic, dentist, and social services.
Health Education
CHWs provide education to the community to promote general health.
